Across England, local health and care teams are working hard to help people stay well at home and avoid unnecessary hospital stays. Last year, Newton and the national Better Care Fund brought together\u00a0<\/span>\u00a0<\/span>leaders\u00a0from\u00a018\u00a0different\u00a0systems across\u00a0health and social care\u00a0to learn from one another and strengthen a type of\u00a0short\u00a0term\u00a0support called\u00a0<\/span>homebased intermediate care<\/span><\/i>.<\/span>\u00a0<\/span><\/p>\n

This support focuses on helping people recover at home after an illness or injury or avoid going into hospital in the first place. It might involve rehabilitation, practical help around the home, or short bursts of intensive support.\u00a0Ultimately, the\u00a0goal is simple:\u00a0<\/span>help people regain confidence and independence in the place they\u00a0tend to\u00a0feel most comfortable \u2013 their own home<\/span><\/b>.<\/span>\u00a0<\/span><\/p>\n

Why this matters<\/strong><\/span><\/p>\n

We know from national evidence that people\u00a0generally\u00a0recover\u00a0better when they can stay at home. They\u00a0return to health\u00a0more quickly, are less likely to need long\u00a0term care, and avoid the disruption of unnecessary hospital time. This approach also helps free up hospital beds for those who need them most.<\/span>\u00a0<\/span><\/p>\n

We\u2019ve seen the difference this kind of work can make in places like Leeds,\u00a0where\u00a0Newton\u00a0worked directly with the Health & Care Partnership to redesign how intermediate care operates,\u00a0resulting\u00a0in\u00a0169 more people each year returning home rather than moving into long<\/span>\u2011<\/span>term beds,\u00a0an 8.2day reduction in short\u00a0term bed stays,\u00a0and 421 more people going straight home from hospital.<\/span>\u00a0<\/span><\/p>\n

Every winter, the NHS experiences a sharp increase in demand;\u00a0more people need urgent care, hospitals become busier, and local services come under significant pressure. This initiative was designed to help local health and care systems strengthen their support at home so they could manage these winter pressures more effectively.<\/span>\u00a0<\/span><\/p>\n

Where we started<\/span><\/strong><\/p>\n

Over four sessions, teams from across the country explored what \u201cgood\u201d looks like in homebased intermediate care. Together, they:<\/span>\u00a0<\/span><\/p>\n

  • Clarified the most important problems\u00a0for them\u00a0to solve\u00a0locally<\/span>\u00a0<\/span><\/li>\n<\/ul>\n

    • Agreed the measures that matter\u00a0for them and\u00a0their residents\u00a0<\/span>\u00a0<\/span><\/li>\n<\/ul>\n

      • Built practical,\u00a0locally-specific\u00a0plans focused on making meaningful improvements before winter<\/span>\u00a0<\/span><\/li>\n<\/ul>\n

        • Committed to delivering at least one real, tangible change that would\u00a0benefit\u00a0local people quickly<\/span>\u00a0<\/span><\/li>\n<\/ul>\n <\/div>\n<\/div>\n\n <\/div>\n <\/div>\n <\/section>\n\n \n\n\n

          What changed on the ground <\/h2>\n
