Leeds HomeFirst: collaborative partnership working and its impact on people’s outcomes
For the last year and a half, the Leeds Health and Care Partnership has been delivering ‘HomeFirst’, a bold and innovative programme aiming to achieve a sustainable, person-centred, home-first model of intermediate care that is joined up and promotes independence. In this session, system leaders will reflect on this transformation journey and describe how cross-system working at all levels has been a core enabler to the incredible impact for people and the system, this work has achieved.
